Too many vulnerable people miss medications and appointments, face occupational threats, get exploited, or otherwise face unscrupulous hazards due to gradual isolation from society.  This isolation may be caused by dementia, mental stress, or the general plight of a family moving away from home, resulting in a real risk to them without deliberate checks by authorities.
